Abstract

Patients who are going to undergo surgery often experience anxiety due to the uncertainty of the surgical procedure, pain, or potential complications after surgery. One non-pharmacological intervention used to reduce anxiety is instrumental music therapy. This study aimed to examine the application of instrumental music therapy on anxiety levels in pre-operative laparotomy patients at Dr. Tajuddin Chalid Hospital. Method: A case study on patient Mrs. S who was going to undergo a laparotomy. Anxiety levels were assessed using the Hamilton Anxiety Rating Scale. The intervention provided was Mozart Symphony No.40 instrumental music therapy for 15 minutes at a volume of 20 dB, given once. Nursing care results before the intervention showed the patient had severe anxiety with a HARS score of 34, marked by complaints of anxiety, fear of facing surgery, worry, and appearing restless and tense. After receiving instrumental music therapy, there was a decrease in anxiety levels to mild anxiety with a HARS score of 14. The patient appeared calmer, more relaxed, and able to manage anxiety before the surgery. In conclusion, instrumental music therapy using Mozart's Symphony No.40 is effective in reducing anxiety levels in pre-laparotomy patients. This intervention is one of the non-pharmacological nursing approaches to help manage anxiety in pre-operative patients.
